Does this tool make the hole or just fasten the eyelets (or both)?

It punches the smaller hole but not the 1/4 inch which is a huge disappointment for me. I still need to find a easier way to make 1/4 hole than using scissors.

Does it work on leather?

I used it on pleather, not real leather and it worked well. I'd bet it would work on a relatively thin leather but not anything thick and you'd need something sharper to poke the holes.

Would this work to punch holes and apply eyelets on thick wool fabric (coat fabric)?

I haven't used it on wool, but it worked on 3 layers of a medium thickness fake leather fabric pretty well. The hardest part was actually getting it to the right place in the fabric because of how short the nose is.
